Transition

1. Explanation of the goal:

Transition is the process of youth with disabilities going from school to adult life.

A successful transition is one in which young people live, work, and are involved in inclusive settings of their choice, rather than being "placed" in what's available.

2. The change we want to see:

Young adults with significant disabilities will make successful transitions to jobs and activities of their choice in the community.

3. How we will know the change happens:

The Council will monitor the progress of each objective to learn how many young people are moving successfully from school to community life.

4. What we’ll do to get there:

This part of the Plan has three objectives or activities that target education and two objectives or activities that target employment:

Education Objectives:

  1. By September 2011, 20 elementary schools will have developed a culture of inclusion.
  2. By September 2011, 100 youth with disabilities will set goals for community jobs and activities and outline a plan to reach the goals.
  3. By September 2011, 100 youth and adults with disabilities will assert their choices to family, professionals and others.

Employment Objectives:

  1. By September 2011, 50 young adults with significant disabilities will go from high school to jobs of their choice in the community.
  2. By September 2011, 30 people with developmental disabilities will be newly self-employed or work in a consumer-run business

5. Progress to date (as of August 2008):

Educational Objectives:

Objective 1:

  1. Four elementary schools team with KIDDO/ARC to develop inclusive projects.
  2. Six inclusion leaders attend National Institute on School Inclusion conference.
  3. Work group has started to produce a tool kit to market school inclusion project.
  4. Board contracts with Arts @ Large to work with 18 schools in Milwaukee and 2 out state schools (Eau Claire and La Crosse).
  5. Provided grants to schools to personalize inclusion programs.
  6. Provided promotional resources to promote inclusion in elementary schools.

Objective 2:

  1. Produced Peer Power DVD, brochure, and tool kit.
  2. Working with marketing firm to develop a marketing plan.
  3. Board contracts with Independent Living Council (ILCW) to administer Peer Power. They hire a full-time coordinator.
  4. Helping local groups to develop sustainability.
  5. Developing a network of Peer Power groups.

Objective 3:

  1. Collaborating with YIPPE (Youth in Partnership); 20 youth are identifying career goals, finding jobs, being more assertive.
  2. Curriculum has been completed; distribution to follow soon.
  3. Provided assertiveness training and funding for a 2nd year.
  4. 20 more youth are in the 2nd group.

Employment Objectives:

Objective 1:

  1. Three youth found jobs working with YIPPE.
  2. Staff participate on Managed Care and Employment task force. Four public hearings held and recommendations developed.

Objective 2:

  1. Provided technical assistance on creative employment options and natural supports.
  2. Staff participate on Managed Care and Employment task force. Four public hearings held and recommendations developed.
